	Insurance Claims: We submit initial claims to your insurance as a patient courtesy. Your insurance is a contract between you and your carrier. It is your responsibility to make sure that the services you receive are a covered benefit. We will do all we can to help in repayment by your insurance carrier, yet it is your responsibility to see that payment is received. You may be required to pay a deductible or copay at time of service; monthly statements will be sent to all patients except medicaid patients. Payment for services should be made within 60 days.
